Element TYPE is undefined in FORM error

I just tried my form again and it is working. Can you give me any information why I'd get the error below and then it would work? We've been using this same form since we went live without error. Today we began receiving - Element TYPE is undefined in FORM. <br>The error occurred on line 290. The code on line 290 is within the form to generate email. Adding Windows media files

I have set up a content object that will play a WMA file. The problem I am having is in uploading the file into the Savvy directory. I have added WMA to the allowed file formats to both image and documents, but they will not upload. They are under the 1mb size limit. If I upload via FTP, everything works. Since I ultimately won't be the one uploading the files, I need the ability to upload via Savvy.See More
